Carmex Printable Rebate Form ~ Try Me Free at CVS ~ Expires 12/15/2011

Simply CVS reader Pat shared the other day about increasing her spending power at CVS by utilizing rebates. She mentioned a current rebate form for buying Carmex at CVS.  I wanted to share it here for those who are interested.  The regular price is above the amount of the rebate--but it doesn't expire for a couple of months so we can watch for a sale.  Here are the details from the Carmex Rebate form:

To receive your rebate offer, up to $2.99 for the purchase of (Carmex Moisture Plus ultra hydrating lip balm) you must:

• Buy one specially marked Carmex Moisture Plus ultra hydrating lip balm between 09/01/11 and 12/15/11 at CVS. 
• Mail in this form along with the original UPC from product package and the original CVS store cash register receipt, dated between 09/01/11 and 12/15/11 with your product purchase and price circled.

There is also a $.50/1 printable coupon for liking their Facebook Page.  I printed it and there were no numbers under the UPC symbols--not sure what that will mean when I try to use it. (Thanks to theGeers05 on We Use Coupons for the heads up on the coupon).
